Map of Long Term Care Planning Grants
The Department of Health and Family Services has awarded grants to ten groups for the purpose of carrying out planning activities to explore the expansion of the Family Care program and promote the integration of long-term and health care services.
The Request for Information/Proposals were designed to identify potential partners (private and public) to help the State plan and implement reform of health care and long-term support. Attached below are the RFI and RFP submitted by the Southwest Wisconsin Care Management Coalition.
